We accept most major PPO and HMO insurance plans. We also offer self-pay options. Please call our office and we will verify your specific coverage before your visit.
No referral is required. You can schedule an appointment directly. However, if your insurance plan requires a referral for coverage, please check with your provider.
Our office is located at 11914 Justice Ave, Baton Rouge, LA 70816. We are easily accessible and have parking available for all patients.
Please bring a valid photo ID, your insurance card, a list of current medications and supplements, and any relevant medical records if you have them. A completed intake form will also be provided at check-in.
We accommodate urgent care needs when possible. Call our office as soon as you need care, and we will do our best to fit you in the same day. For true emergencies, please go to the nearest emergency room or call 911.
